Job Summary : The chief responsibility of the Floating Residential Services Coordinator is to provide services when there is a vacancy in a staff position, or to provide additional support at a site where appropriate/assigned. This position will provide collaborative, person-centered recovery and goal planning with residents/participants utilizing the recovery model. These services also include keeping accurate records, coordinating with supports, as well as working as an advocate for the resident in connection with a team.

 

Essential Duties and Responsibilities

Core Responsibilities:

·       Build relationships and foster a sense of community. Encourage Residents to participate and support various functions and opportunities at Project HOME.

·       Acquire and maintain a comprehensive knowledge of the mental, physical, emotional, educational, social, medical, financial, employment and education status of each Resident.

·       Design and implement recovery plans in the areas of recovery, health care, medication monitoring, money management, education, and employment which encompass short and long term goals, action plans and means for evaluation and revision.

·       Assist Residents with learning to maintain lease obligations, independently or with the help of supports.

·       Work with Residents and supports through crises, assessing the situation and making necessary plans/interventions. Provide ongoing service coordination and support. 

·       Become familiar with all emergency procedures.

·       Observe confidentiality, privacy, and dignity of each resident.

 

Service Coordination:

·       Maintain case records and reports as required in a timely and thorough manner including case notes, quarterly recovery plans, Project HOME and HUD assessments. Utilize ETO database for all documentation.

·       Contribute to fostering an environment that combines the necessary structure, case management and/or service coordination support, counseling, interaction, freedom, and safety to facilitate Resident growth, individuality and connection to the Project HOME and the external community.

·       Provide ongoing consultation with individuals in recovery and/or behavioral health concerns.

·       Coordinate services and linkages with outside agencies/providers.

·       Seek out and participate in all relevant trainings and in-services.

·       Foster and model respect, empathy, and understanding of Residents.

 

 Assist Residents with coordination of benefits and resources:

·       Refer Residents to BenePhilly, and work in partnership with BenePhilly and outside providers to support Residents in filing for benefits or resolving any issues through DPW, SSA, unemployment, etc.

·       Assist Residents in the job search process as needed and provide referrals to Employment Services program.

·       Assist Residents in coordination of their team supports, including ICM/TCM supports, PRS, peer, and employment services.

·       Assist Residents with medical and behavioral health referrals and communication as needed.

·       Support Residents through the completion of housing recertification process and other housing related needs through collaboration with Property Management. Partner with Property Management and Residents on building and unit inspections; frequency varies depending on Residential Program/Site policy.

 

Meet as scheduled with the following:

·       Participate in monthly staff and resident meetings, and special committees where applicable.

·       Meet weekly in supervision with site Program Manager and attend team meetings; meet with Floating Program Manager monthly/as needed.

·       Participate in trainings and continuing education opportunities.

·       Facilitate weekly Wellness Self Management groups (where applicable) and individual sessions with Residents bi-weekly/as needed based on the individual needs and goals of the resident. Participate in activities as needed.

·       Schedule and facilitate collaborative linkage meetings with internal and external supports on a regular/as needed basis.

 

Intake/Orientation/Discharge:

·       Utilize Critical Time Intervention best practice with all chronically homeless incoming residents and others as needed.

·       Complete all admission and discharge paperwork and assessments. Orient new resident to residential sites and neighborhood using Resident Handbook and Resident Orientation process.

·       Coordinate resident discharges.

 

Alternate on-call responsibilities with other residential staff for emergencies. Report and respond to incidents in a timely and appropriate manner.

 

Minimum Qualifications

·       BA/BS degree with 1 year, or, 3 years minimum related work experience with mental illness/dual diagnosis, and supportive housing.

·       Experience working with homeless population; substance use/persons in recovery.

·       Very strong verbal and written communication, organizational, and interpersonal skills.

·       Team experience

·       Strong computer literacy

·       Valid PA driver’s license

Preferred Qualifications

·       Knowledge of Philadelphia behavioral health, substance use/abuse, and housing systems, services, and resources.

·       Knowledge of crisis intervention, de-escalation

·       Group facilitation experience

·       CPR/First Aid certified
