A New Leaf’s La Mesita Family Homeless Shelter is a 16-unit family shelter providing a broad array of services to help homeless families with minor aged children become self-reliant. It is the only shelter in the East Valley that allows parents to bring their children with them, a critically needed resource for families in crisis. Families stay up to 120 days in 1 or 2 bedroom apartments with basic needs provided upon entry including food, clothing, hygiene and household items. 

We are seeking to fill a full time Intake Specialist position. 

Schedule: Monday-Friday, 8A-5P

Pay Rate: $15.00/hour
Location: Mesa, with travel to Surprise in the upcoming months 


Responsibilities will include but not limited to:

  • Complete diversion and enter outcome information into HMIS. If needed, complete VI-SPDAT and enter into HMIS as well.       
  • Complete intake paperwork with all incoming clients; including, assess/develop interim service/safety/crisis plans and perform lethality/basic needs assessments.                        
  • Identify immediate needs and provide appropriate resources to address needs; including, providing any referrals or transportation as needed, especially if shelter is not available.      
  • Provide crisis intervention as needed and necessary.                         
  • Coordinate client appointments/ schedule initial assessments with the case managers.
  • Provide initial tour to family at intake of the new assigned unit.
  • Track and document after care findings for 90 days once family has exited the program.        
  • Participate in required client care supervision on a regular basis as required by state law and A New Leaf policy.
  • Complete documentation/paperwork in a timely and accurate manner.                                               
  • Provide administrative support and functions as requested.
  • Facilitate resident meetings with the case managers routinely.

  • Travel between family shelters located in East and West valley to conduct intakes of new families.
  • Report to management data on intakes, exits and housing outcomes in monthly report.
  • Drive agency vehicle to transport clients.
  • Drive personal vehicle and agency vehicle to conduct program business.




  • High School or GED required.
  • Associate or Bachelor's degree, preferred.


  • Experience in case management/knowledge of community resources preferred.
  • Experience in social service related field required.  
  • Bi-lingual/Spanish speaking preferred.     


  • Class One Fingerprint Clearance Card
  • Valid Driver’s License with reliable transportation
  • Current Auto Liability Insurance
  • Must be 21 years old due to licensing requirements
  • 39 Month Motor Vehicle Record
