A New Leaf’s East Valley Men’s Center (EVMC) provides shelter and support services to single men in our 100+ bed facility. We strive to provide a hand up and not a hand out at the East Valley Men’s Center. Our program is considered a “working man’s facility,” requiring and assisting men to secure employment and save money for their transition to safe, stable housing.


We're looking for a full-time Case Manager who will assist the program and the clients by providing a supportive and structured environment to assure safety and respect.   


Full Time Schedule: Monday-Friday, 8AM-5PM, some evening hours flexibility may be required

Pay Rate: $15.00 per hour     

Duties and Responsibilities:

  • Provide case management services to homeless clients; establish/maintain client files.         
  • Directly responsible to Program Manager for issues regarding admission and discharge, while being in conformance with the agency’s program definition and policies as relevant to program/client needs.
  • Screening, assessing and developing service plans, and monitoring the implementation of strength-based case plans and progress towards goals.                      
  • Assisting clients in obtaining community services and benefits, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies/services.
  • Providing referrals and/or coordinating transportation for clients to community services as needed (as relevant to program/client needs).       
  • Facilitating and coordinating discharge planning transition and follow up according to plan.    
  • Recognizing common symptoms of mental disorders and/or addictions.         
  • Recognizing the indications, common side effects, reactions and interactions of psychotropic medications prescribed for self-administration by agency clients.               
  • Completing accurate case documentation and agency paperwork in a timely, correct, and thorough manner.
  • Coordinating services with clients/families and other service providers.                                  
  • Maintaining client demographic information and reporting this information to Administrative Assistant / Program Manager.
  • Providing educational information as needed to assist clients/families in reaching goals.        
  • Attending and participating in court hearings as required to meet program needs and within scope of practice.
  • Performing intake screenings, lethality assessments and basic needs assessments for potential clients (as relevant to program/client needs).
  • Schedule client appointments and obtain required authorizations (as relevant to program/client needs).
  • Assist in the translation of barriers and successes into positive system changes (as relevant to program/client needs).



  • High school diploma or GED required.
  • 2 years experience working with homeless or economically disadvantaged population required.
  • Knowledge of community resources
  • Due to state licensing, applicants must be a minimum 21 years old.
  • Demonstrate and provide strong communication skills both verbally and in writing.
  • Committed to teamwork and have the ability to establish and maintain effective working relationships. Demonstrate a basic understanding of Trauma Informed Care.
  • The skills necessary to engage and work with others from diverse ethnic and cultural groups, coupled with maintaining professionalism at all times.
  • Able to maintain proper professional and personal boundaries, while protecting our Resident’s Rights.
  • Possess a valid driver’s license with reliable transportation, current auto liability insurance and a clean 39-month motor vehicle record.
  • A Class One Fingerprint Clearance Card or be able to pass various background checks in order to obtain one.
