A New Leaf’s Dorothy B. Mitchell Counseling Center (DBMCC) offers community-based behavioral health services for youth, adults, and families.

Counseling, case management, medication management, and support services are provided throughout Maricopa County. 
Schedule: Monday-Friday, 8AM-5PM,  Full Time 

DUTIES AND RESPONSIBLITIES:  Responsibilities will include but not limited to:


  • Reports to clinical supervisor for administrative and clinical issues regarding case management duties, while being in conformance with accepted clinical practice and the agency's program definition and policies, relevant to program/client needs. Participates in a minimum of 4 hours per month of clinical supervision.                            
  • Coordinating Child and family Teams (CFT) and serving as a facilitator and/or team member, using CFT practice guidelines and ensuring that the Plan is carried out.               
  • Becoming skilled in building/supporting Child and Family Teams or any alternate collaborate approach.       
  • Developing and maintaining collaborative relationships with clients, their family members and significant support systems. Meeting frequently with clients and family members in their natural setting; providing face to face and telephone crisis interventions (while under appropriate clinical supervision).
  • Screening, assessing, develop, and implement treatment/service plans under the direction of the Team.
  • Ensuring that treatment/service planning will include a plan for discharge and estimated date.     
  • Co-developing 24/7 Safety/Crisis Plans with every assigned client. This play may be simple or complex, depending on the risk factors presented by the client.                               
  • Updating and completing staffing notes for multi-disciplinary clinical team reviews.        
  • Developing/Monitoring/Evaluating the implementation of Strength-based Family plans and progress of family towards goals.                                
  • Collaborating with Child and Family Teams, arranging for appropriate evaluations, medication monitoring and referrals and community based services.                             
  • Coordinating comprehensive support though in-home and community based education and interventions to address the families' goals.                          
  • Assisting clients and families in obtaining community services, securing appointments with outside services and agencies as needed, including identification of funding sources and linkages to other agencies/services.                  
  • Coordinating transportation for clients and family members as deemed medically necessary and in accordance to agency policy.                         
  • Supporting the most appropriate and least restrictive education setting inclusive of participating in Individual Education Plan (IEP) meetings.                           
  • Assisting clients/families and other team members in identifying and resolving barriers to the implementation of their service plan.                                    
  • Facilitating and coordinating discharge planning transition and follow up according to plan.        
  • Recognizing common symptoms of mental disorders and/or addictions.                             
  • Recognizing the indications, common side effects, reactions and interactions of psychotropic medications prescribed for self-administration by agency clients.
  •  Completing accurate case documentation and agency paperwork in a timely, correct and through manner.          
  • Coordinating services with clients/families and other service providers.                             
  •  Maintaining client demographic information and reporting this information to program coordinator/manager.
  • Providing psycho educational information as needed to assist clients/family in reaching goals.                 
  • Helping families to make a smooth transition from the child system to the adult system.   
  • Attending and participating in court hearings as required to meet program needs and within scope of practice.         
  • Receive clinical oversight as required by state law and A NEW LEAF policy and procedures.      
  • Schedule client appointments.                                   
  • Attend and participate in case management and/or AZ Practice Model Trainings               



• Associates Degree in Non-Behavioral Health field

• Three (3) years behavioral health experience

• Associates Degree in Behavioral Health field

• Two (2) years behavioral health experience

• Bachelor's Degree in Non-Behavioral Health field

• Two (2) years behavioral health experience

• Bachelor's Degree in Behavioral Health field

• One (1) year behavioral health experience



  • Class One Fingerprint Clearance Card
  • Valid Driver’s License with reliable transportation
  • Current Auto Liability Insurance
  • 39 Month Motor Vehicle Record
