Hope Haven is an accredited resource for disability services and advocacy. We offer service programs and support to meet a broad range of needs for children, adults, and families – from physical, mental, and intellectual to residential, vocational, and spiritual.
Primary Duty: The QIDP Supervisor is responsible to the ICF/ID Manager for the coordination of individual service plans for the assigned caseload of persons served in the ICF/ID program.  The QIDP Supervisor will supervise the QIDPs and Recreation Coordinator. This position will also be required to cover direct care shifts when staffing patterns require it.

DUTIES AND RESPONSIBILITIES:
  1. Supervise the QIDPs assigned. 
  2. Cover DSP shifts as scheduled.
  3. Be a lead trainer for the behavioral management system and the communication systems used by the organization.
  4. Work with the Residential Lead to assure client programming is being implemented as written in the ISP.
  5. Ensure training is being done consistently and also make timely modifications to facilitate achieving desired skills or goals by identifying inconsistencies in training procedures and facilitate resolution of inconsistencies.  
  6. Serve as liaison with the consultants.
  7. Serve as administrative on call.
  8. Assist with the staff scheduler as needed. 
  9. Conduct and follow through on quality assurance measures as it pertains to programming and active treatment.
  10. Serve as an administrator to the EHR.
  11. Serve as an administrator of residential services
  12. Maintain compliance with the Departments of Inspections and Appeals (DIA) regulations and participate in the annual survey, self-reporting, and plan of correction process.
  13. Oversee all quality assurance systems for the programming department. (Competency audits, Quest Surveys, shift audits, staff training, QIDP files, etc)
  14. Attend IDT meetings, home meetings, and ICF/ID team meetings.
  15. Work with ICF/ID Manager to create and maintain efficient systems within the homes and in the programming department.
  16. Responsible for monitoring resident aggressions, and collaborating with DIA for investigations and plans correction.
  17. Serve as lead of the Human Rights Committee to monitor activities to ensure least restrictive environments for those individuals receiving services. 
  18. Serve as a member of committees as assigned by the ICF/ID Manager, including placement.
  19. Coordinate services and programming with the resident’s schools and other provider agencies to facilitate an interdisciplinary program for each resident served.
  20. Along with QIDP’s update and maintain resident’s adaptive and therapeutic equipment supplies, devices and equipment.
  21. Coordinate with the QIDPs regarding out of facility consultations, evaluations and assessments for individuals served, as recommended by the IDT.
  22. Serve as a member of the Consumer Advocate Team when requested by the Consumer Advocate Team Chairperson.
  23. Review, track and trend incident reports.
  24. Inform ICF/ID Manager of all relevant departmental issues in a timely manner.  
  25. Be the contact for new referrals and initiate the review process to determine if the child is a good placement or not. 
  26. Assure all the contacts have been made, discharge information is complete and sent to the necessary parties prior to the actual discharge of a child. 
  27. Develop and Schedule staff trainings that are needed to maintain the necessary Active Treatment and Safety needed in the homes. 
  28. Assure staff trainings are up to date and completed. This includes all Mandatory Reporter, CPR, First Aid, and all CDS trainings assigned. 
  29. Teach the RSSG class as needed.
  30. Become Sensory Trained and become a leader in training others in Sensory Techniques. 
  31. Other duties as assigned.

PHYSICAL REQUIREMENTS:
  1. Requires physical mobility to walk, stoop, stand, sit and climb steps.
  2. Requires the physical strength to assist individuals in transferring as per Hope Haven policy.
  3. Requires physical strength to restrain an individual as outlined in the SCM (Safe Crisis Management) procedures.
  4. Ability to drive  
  5. Ability to lift 50 pounds.

QUALIFICATIONS:
  1. BA degree in Human Services related field or a license as a Registered Nurse.
  2. Possess a minimum of one year experience specific to working with persons with Intellectual Disabilities and/or other developmental disabilities.
  3. Supervisory experience
  4. Good communication (verbal and written), organizational and leadership skills.
  5. Computer literate or have basic knowledge computer skills.
  6. Ability to exercise sound judgment in making decisions.
  7. Ability to complete RA courses and/or required orientation training and all other agency requirements.
  8. Valid Class C driver’s license required, unless driving a vehicle with a capacity of 8 or more, then a Class “D” license will be required and be an insurable driver with our insurance company.
  9. Ability to pass a health assessment at time of hire and ability to maintain up to date health assessment as requested per regulations.
  10. Meet DOL standards for sedentary work with occasional Light work.
  11. Live within 30 minutes driving distance of FHC preferred.
  12. Complete training in Child/Dependent Adult Abuse Reporting within the first six months of employment and on a continuing basis as required by Iowa law and Faith, Hope and Charity policies and procedures.
  13. Clearance from a criminal background check and abuse reporting check at time of hire.
  14. Negative drug screening.
  15. Reliable Transportation.
  16. Provide proof of state minimum liability insurance coverage.
  17. Ability to see work needed to be completed and delegate it to the best person to complete the task.
  18. Ability to work with children with mental illness, intellectual disabilities, and behavior concerns, providing support in all aspects of daily living as directed by each person’s individual plan.
  19. Work as a Team Player.
