The Phlebotomist is responsible for setting up lab for day to day operations to include but not limited to specimen procurement, specimen processing, and result reporting. Performs lab test according to lab scope of practice (CLIA waived testing). Responsible for lab maintenance and ordering supplies. Responsible for equipment calibration and quality controls as applicable. 

Essential Duties and Responsibilities:

1.  Keep inventory of supplies and dispose of them according to policies when supplies are expired

2.  Have knowledge of lab policies and procedures

3.  Draw blood from patients for reference lab, process specimens and package, store and send out test

4.  Process in-house waived test

5.  Process requisitions using Electronic Health Records (EHR)

6.  Keep quality assurance logs of equipment and inventory of supplies

7.  Set up equipment for use in lab; ensures proper operation of equipment, and report operating deficiencies to supervisor

8.  Clean lab facilities and equipment after lab exercises are complete; stores supplies and equipment as applicable

9.  Practice safety, environmental, and/or infection control methods

10. Reconcile outgoing test with “Reference Lab Reconciliation Report”

11. Flag results that are not returned within a reasonable period of time and notify the charge nurse and ordering provider

12. Work closely with reference lab to ensure all test come back

13. Work delinquent deferred orders, brings patients that have pending orders processed 

14. Assist charge nurse process letters, call patients to notify them of the normal/no further action lab results. Document patient notification in health record 

15. Track all fecal occult blood test cards given out, follow with patients to ensure they are brought back

16. Responsible to provide complete information to reference lab when processing requisitions

17. Responsible for process missing information request from reference lab

18. Work closely with in-house billing department and Nursing administration to ensure proper billing

19. Act under patient specific written standing orders

20. Monthly monitoring of expiration dates and disposal of lab supplies

21. Notify charge nurse of test that are positive for items that require morbidity report

22. Relay information from reference lab on critical labs to Provider/Licensed Nurse

23. Other duties as assigned



Minimum Qualifications:

1.  High school education or GED Equivalent

2.  Phlebotomy Certificate

3.  Education for Phlebotomy Certificate

4.  One year experience in health care preferred

5.  All staff that is presently working within a clinic is required to have a Current and Valid CPR and First Aide certification on file with the Human Resources Department. This is for clinical and administrative staff. This new license requirement is mandatory for continued employment, CPR and First Aide certification must be obtained though the American Heart Association. Internet certificates do not comply and for this reason are not valid.
